A new guide from Arosa helps families recognize common gaps and prepare for a safer, smoother transition home after hospital discharge.

DURHAM, N.C., Oct. 1, 2026 /PRNewswire-PRWeb/ -- Bringing a loved one home from a hospital or rehabilitation facility can be a relief, but it can also create unexpected challenges. Care management experts caution that discharge instructions may not fully account for the realities of managing medications, follow-up care, safety and daily routines at home.

According to Centers for Medicare & Medicaid Services, 20 percent of Medicare patients are at risk for readmission follow discharge. Families may suddenly become responsible for coordinating multiple aspects of care while helping a loved one adjust to physical limitations or new routines. Even with a detailed discharge plan, it may not be clear how to put that plan into practice and ensure a loved one is on the mend.

Care management experts identify seven common challenges following discharge:

  • The individual or family does not fully understand the discharge instructions.
  • Medication changes create confusion.
  • Follow-up appointments are difficult to schedule or manage.
  • Changes in health or warning signs are overlooked.
  • Mobility challenges create safety concerns in the home.
  • Family members become overwhelmed by caregiving responsibilities.
  • Communication breaks down among the individual, family and healthcare providers.

Before a loved one returns home, families can ask questions about medications, follow-up care, warning signs and activity restrictions. They can also assess the home for safety concerns and determine who will help with meals, transportation, personal care, medication reminders and other daily needs.

"The discharge plan is the starting point," said Stewart. "The next step is making sure the individual and family have a realistic plan for carrying it out once they are home."
